1. Print it blank, fill it in by hand then mail it in or fax it to us.
2. Fill it in online, print it then mail or fax it. Important: Adobe® Reader® does not have the ability to save the form locally to your computer with data in it. This means that once started, it should be completed.
3. Fill it in online, then use the submit by email button to email it directly to us. Your default email client (ie Microsoft® Outlook® or Outlook Express®) will open with the email already addressed to us and your data as an attachment. If your email client does not fill in the email address for you, please send it to HumanResources@thehometownpharmacies.com.
Click Send and you're done.
